TEAM REGISTRATION FORMS
Forms 1-4 must be completed and brought with you to each USBA tournament. YOU DO NOT NEED TO MAIL OR FAX THESE FORMS. If you are not a USBA member, you must register as a member prior to participating in any USBA tournaments. Membership & tournament entry can be completed at the same time.

MULTIPLE TEAM DISCOUNTS
The USBA offers the following discounts for teams from the SAME organization that register for the SAME tournament during the regular season.
2 Teams - 5% off
3 Teams - 7.5% off
4 Teams - 10% off
5 Teams or more - 15% off
If you qualify for a discount, please send an email to info@usbahoops.com. Once your discount is approved we will send a request for the fee via PayPal.com to pay by credit card or you can send the fee by postal mail. There are no discounts for Nationals.
The USBA will determine discounts for multiple teams from the same organization for State, Regional & National events and post them on the website.
